FYI: If you are pre-arranging your services, it’s a great idea to provide this information before death to make things easier on your survivors.

All of the forms below can be downloaded and emailed to us at forms@woodlawnfh.org or faxed to us at 360-438-6881, or just bring them in and give them to your Funeral Director.

It is OK if you don’t know all of the information, just complete as much as you can.

Everyone needs to fill out the Death Certificate Input Form.

All Cremation Clients – The legal Next-of-Kin must sign a Cremation Authorization before the cremation can take place.  If pre-arranging, you may sign your own.

All Embalming Clients – The legal Next-of-Kin must sign an Embalming Authorization before embalming can take place.  If pre-arranging, you may sign your own.
